Enrollment requirements:
- This online degree completion program is designed for students who already have some college credits or a liberal arts-based associate degree. All 21 courses of the curriculum are required. Because of the unique nature of the program, all other courses may not be substituted for courses in the HWM curriculum, and there are no electives.
- Admission Requirements: You are eligible for admission to this degree if you have completed at least 60 semester credits of transferable coursework with a 2.0 or better grade point average (GPA).
- Application: Because the Health and Wellness Management, BS is an online program, you must apply by using the University of Wisconsin System Online Admission Application https://apply.wisconsin.edu. Indicate UW-Stevens Point as your home campus and send your transcripts to the UW-Stevens Point Office of Admissions and Recruitment. - All applicants must have a 2.0 GPA from prior college coursework. To graduate with the Bachelor of Science degree in Health and Wellness Management you must complete all 21 HWM courses, all UW-Stevens Point general education requirements, and achieve an overall GPA of 2.5.

Definition: The online University of Wisconsin Bachelor of Science in Health Information Management and Technology (HIMT) is a degree completion program designed to equip you with the skills to work and lead in the health data management field. Health information professionals maintain, collect, and analyze the data that doctors, nurses, and other healthcare providers rely on to deliver quality healthcare. They are experts in managing patient health information, administering computer information systems, and coding diagnoses and procedures for healthcare services provided to patients.
HIMT is a collaborative, on-line degree. The curriculum for HIMT was developed by faculty at the four partner campuses, UW-Stevens Point, UW-Green Bay, UW-Parkside, and UW-La Crosse, to ensure curricular consistency and quality, and to utilize the academic strengths of the partner institutions.
Accreditation: UW-Stevens Point HIMT program is accredited through the Commission on Accreditation for Health Informatics and Information Management Education (CAHIIM).
Program Description of Health Information Management and Technology
The curriculum consists of 21 classes (61 total credits). Courses cover such topics as health and medical terminology, medical ethics, information technology in healthcare, health benefit plans and providers, and leadership and change management in healthcare. The HIMT curriculum offers two tracks: Health Information Technology (HIT) and Health Information Management (HIM) . The HIT track prepares students for information technology roles in the healthcare, health insurance, and governmental fields. It’s the foundation for positions from health data analyst to insurance claims coordinator. The HIM track prepares students to be leaders in providing better patient care, administering computer information systems and cost, and managing patient information relied upon by health professionals. You will take 17 core courses and then, depending on which track you choose, you will take four additional courses in a given track to complete the degree.
Academic Standards for Health Information Management and Technology
To be admitted:
- You must complete at least 60 undergraduate credits, be computer literate, and have a minimum 2.0 GPA.
- If you wish to obtain a Bachelor of Science in HIMT, you must have completed all general education requirements.
- Application: You must apply to the home campus of your choice who is offering the degree (i.e., UW-Stevens Point, UW-Parkside, UW-Green Bay, and UW-LaCrosse). For complete information on how to apply visit www.himt.wisconsin.edu.
- The UW-Stevens Point School of Health Sciences and Wellness will work in conjunction with UW-Extended Campus advising to help you complete your degree. Application materials, procedures, dates, etc. are available at www.himt.wisconsin.edu.
- Transfer students: you must have a 2.0 GPA from your prior institution and have completed all required general education requirements at the campus with whom you are choosing to affiliate.
- If you are not officially accepted into the program, you will not be allowed to take upper level HIMT courses.

Definition:
The health science and wellness major at UW-Stevens Point provides you with broad skills and opportunities for a variety of careers within the health field. The interdisciplinary major will prepare you for the art of working with people and the science of health and disease.
In addition to core requirements, students may choose pathways to careers or graduate programs, such as: Pre-Athletic Training, Pre-Occupational Therapy, Pre-Physical Therapy, Health Coaching, Worksite Wellness, Healthcare Administration, or Strength and Conditioning.
Academic Standards for Health Science and Wellness Major
For continued registration as a Health Science major you must have a grade point average of 2.75 overall and in the sciences, including biology, chemistry, mathematics, and health science. You are screened for acceptability to apply for internship positions.
Grades in required courses taken prior to declaring academic forgiveness or in repeating courses will be used in calculating your grade point average. No more than two courses with grades of less than C- will be accepted in the major. If you do not meet these standards, you may petition the director for consideration. The minimum GPA for graduation is 2.50.

Definitions:
Athletic Trainers are health care professionals who provide service and treatement in the areas of injury/illness prevention, emergency care, clinical diagnosis, therapeutic intervention and rehabilitation of injuries and medical conditions.
The Master of Science in Athletic Training (MS-AT) degree is a professional athletic training program that prepares you for certification as an athletic trainer. There are two tracks for admission: Traditional - completed undergraduate degree or the accelerated Athletic Training 3 + 2 Option (BS in Health Sciences and Wellness, MS in Athletic Training).

Academic Standards for Master of Science in Athletic Training
Admission to the Program: All applicants applying for the MS-AT program must complete the following:
- UW-Stevens Point application for graduate study.
- Application to the MS-AT graduate program via ATCAS Central (www.caate.net/apply-now). The ATCAS application requires:
- a written essay,
- two letters of recommendation,
- evidence of 20 hours of athletic training observation with a certified AT.
- a listing of complete or in-progress prerequisite coursework, and
- completion of the Technical Standards Form for the MS-AT program.
- TOEFL or IELT scores (international applicants only). Minimum of 80 or 7 respectively.
- Official transcripts of all college work.
- Attain a minimum cumulative GPA of 3.0 on all coursework prior to application. If your GPA is under 3.0, you can still apply and request a review of your application.
Prerequisite courses for the MS-AT program are: biology, chemistry, physics, human anatomy, human physiology, exercise physiology, kinesiology, psychology, nutrition, medical terminology, and statistics. Additional Notes about Applying to the Master of Science - Athletic Training (MS-AT):
- Application deadline: Application deadlines for early admission and the regular decision cycle are published on the MS-AT website at www.uwsp.edu/msat
- Qualified applicants will be contacted by the AT Admission Committee upon receipt of completed application. Meeting or exceeding the minimum requirements for eligibility to apply does not guarantee acceptance into the MS-AT.
- A final review of applications will be conducted prior to the start of the first summer term in the MS-AT to ensure any outstanding coursework was completed and admission requirements are met.
Upon Acceptance to the Master of Science - Athletic Training (MS-AT), you must:
- Provide verification of current physical examination by an approved health care provider (MD, DO, PA, CNP) confirming you meet the physical and mental abilities required of an athletic trainer.
- Provide current records of: immunization history (including MMR, Hepatitis B, Varicella, adult dTap); tuberculosis (TB) test within the past 12 months.
- Provide documentation of annual TB test updates.
- Obtain antibody titers and/or influenza vaccine depending on requirements of assigned clinical sites.
- Complete a healthcare provider background check as required by regulations in Wisconsin Administrative Code Chapter HFS 12.
- Supply verification of personal healthcare provider liability insurance.
- Submit all health-related documentations/verifications to the Clinical Education Coordinator prior to the start of each semester.
Retention, Master of Science -Athletic Training (MS-AT)
In order to maintain good standing within the MS-AT, you must:
- Maintain a 3.0 GPA in major.
- Maintain 3.0 average evaluation score (4-point scale) on clinical evaluation. If you fail to meet either criteria listed above, you will be on academic probation in the MS-AT for one semester. If you do not attain the retention criteria during the probation semester, you may not continue in the major. You are only allowed one probationary semester during your time in the MS-AT.
Notes for Master of Science -Athletic Training (MS-AT) Students
- All requirements for the AT program must be complete, or in the process of being completed, before the program director will endorse your application for the Board of Certification examination.
- All students in the MS-AT are responsible for arranging travel to off-site clinical rotations. You will also be responsible for the cost of travel. There will also be some additional expenses incurred for uniforms and learning resources.
- Updated information will be provided in the Athletic Training Educational Handbook and the program website.
- If you are denied acceptance into the program, you may contact the assistant dean of the School of Health Sciences and Wellness for appeal procedures. Appeals are handled by the School of Health Sciences and Wellness Appeals Committee.

You may earn the MS degree in Community & Organizational Leadership or the MS degree in Sustainable and Resilient Food Systems . In both programs you have an opportunity for graduate teaching and research assistantships as well as for in-depth community practitioner experiences.
Admission to the Program - Community & Organizational Leadership, MS - Graduate Program
For Community & Organizational Leadership you need:
- A baccalaureate degree from an accredited university or college with an undergraduate grade point average of 2.75 or above (based on a 4.0 scale). We may grant probationary status if you have a GPA below 2.75.
- A current resume or curriculum vitae detailing your professional experience.
- A letter of intent detailing your background qualifications, career goals, and how this program fits into your future professional plans. Professional writing ability will be evaluated based on your submitted documents.
- GRE (Graduate Record Exam).
Admission to Candidacy for Community & Organizational Leadership, MS - Graduate Program
Admission to candidacy (program planning) does not occur until after you have applied and are accepted into the Community & Organizational graduate program at UW-Stevens Point. Once admitted, you will consult with your adviser to prepare a proposed program of study. Acceptance as a candidate occurs when your graduate committee has approved your Program of Study.
To be accepted as a candidate:
- Complete no more than 9 credits of graduate work from UW-Stevens Point. Transfer credits may apply toward completion of your degree, but they are not included in consideration for admission to candidacy. You may apply earlier, but you should be admitted to the program and meet with your adviser before you have completed 9 graduate credits.
- Remove any academic deficiencies and/or complete all subject matter prerequisites.
- Maintain at least a 3.0 (B) grade point average in all your graduate work.
- Receive approval for your Program of Study.

The MS-HCA prepares patient-care professionals, health administrators, or management professionals to better accomplish the triple aim of improving access to care, impacting the quality of care, and reducing the overall cost of care in a variety of healthcare settings by increased skills in workforce recruitment and retention, financial management, organizational development, health law and policy, population health, and communication.
The fully online media-rich MS-HCA curriculum was developed by faculty at five partner campuses: UW-LaCrosse, UW-Parkside, UW-Platteville, UW-Stevens Point, and UW-Stout. Instruction may originate from any of these universities to utilize the academic strengths of the partner institutions. The University of Wisconsin-Stevens Point is seeking accreditation of a new physical therapist education program from CAPTE. On November 22, 2021, the program submitted an Application for Candidacy, which is the formal application required in the pre-accreditation stage. Submission of this document does not assure that the program will be granted Candidate for Accreditation status. Achievement of Candidate for Accreditation status is required prior to implementation of the professional phase of the program; therefore, no students may be enrolled in professional courses until Candidate for Accreditation status has been achieved.. Further, though achievement of Candidate for Accreditation status signifies satisfactory progress toward accreditation, it does not assure that the program will be granted accreditation. ¹

Program Description
The DPT Program prepares you for entry-level physical therapy practice. The program is a mix of didactic learning experiences offered through hybrid instruction, augmented with integrated clinical practice and summary clinical internships. To execute its vision of transforming societal health by meeting the needs of our communities, as an entry-level DPT program graduate, you will:
- Demonstrate competence as a direct access practitioner
- Demonstrate effective communication skills with diverse patient populations, professions, and community sectors to provide patient centered and socially responsible care
- Demonstrate professionalism in physical therapist practice consistent with University and APTA Core Values
Application and Admissions to the Doctor of Physical Therapy:
- Application to the DPT program is through the Physical Therapist Centralized Application Service (PTCAS) (http://www.ptcas.org/home.aspx). Applications will open every June. You should be prepared to submit:
- A copy of official transcripts from all institutions from which you have earned college credit;
- Required essays;
- GRE scores. You may submit the scores directly to the University (Code: 1919) or can be submitted directly to PTCAS.
- Admission to the program is competitive and requires:
- Demonstration of Academic Readiness:
- Minimum cumulative GPA of 3.0 or better. Note that ALL courses including retakes count towards the calculation of this GPA.
- Minimum cumulative prerequisite course GPA of 3.0 or better. Required prerequisite courses are found on the DPT Program website (https://www.uwsp.edu/health/Pages/graduate/dpt/admissions.aspx). You are STRONGLY encouraged to have prerequisite courses reviewed prior to application and to submit the Prerequisite Checklist completed prior to application. This document may be uploaded with the PTCAS application.
- No more than 2 prerequisite courses may be incomplete or in-progress at the time of application.
- All prerequisite courses must have been completed within 7 years of the date of application. Request to consider exceptions to this statute should be sent to the program Admissions Committee by program email with a Completed Prerequisite Checklist.
- The program accepts up to 5 courses to meet the prerequisites earned by AP, IB, and CLEP courses for program prerequisites.
- Anatomy and Physiology program prerequisite requirements MUST be completed in biology, neuroscience, anatomy, or integrated physiology department
- Minimum Quantitative and Verbal GRE of 148 or better; minimum written GRE of 3.0
- For international applicants a minimum TOEFL score of 63 for the composite of reading comprehension, listening comprehension and writing and a minimum score of 26 for speaking.
- Demonstration of Attributes and Experiences that are consistent with program expectations:
- Attributes and experiences are entered by the students in PTCAS and evaluated by the Selection Task Force.
- Participation in a campus visit/interview will be considered in final admission decisions.
- Prior to matriculation you must:
- Complete the undergraduate degree and all outstanding prerequisite coursework with at least a C or better and forward official transcripts to the Program
- Complete the Technical Standards Form for the DPT Program (link to this on the program website) and submit to the Program
- Complete the following assurances and forward verification to the Program:
- Current records of immunization history or titre (MMR, Hepatitis B, Varicella, adult dTap TB test), additional immunizations may be required for clinical learning experience;
- Complete a healthcare provider background check; Pursuant to 2017 Wisconsin Act 278 prior to submitting an application for a credential, an individual may apply to the Wisconsin Department of Safety and Professional Services for a determination of whether the individual would be disqualified from obtaining the credential due to his or her conviction record. If you wish to discuss this, you should contact the Program Director;
- Purchase personal professional liability coverage; specific coverage requirements are available on the program website;
- Provide verification of personal health insurance;
- Complete any additional health testing or safety training as required for clinical education participation. The most current requests will be posted on the DPT program website.
Additional notes about applying to the DPT program:
- The program will use a prioritized rolling admission process.
- PTCAS opens applications in June; If you submit a completed and verified application to the program by August 15th you will be considered in the first batch of students considered for admission; you will receive an admission decision before any second group of students is considered.
- If you submit a completed and verified application to the program by January 15th you will be considered in the second applicant cohort. You are encouraged to update your application in PTCAS with Fall grades. You will receive an admission decision before any additional group of students is considered.
- If needed the program will consider you if you submit a completed and verified application by March 1st. Applications not received by this hard deadline for admissions will not be considered.
- If you are recommended for Admission by the Program you must submit an application for Graduate Study at UWSP. You will have 15 calendar days after receiving a recommendation for admission to accept the program offer and complete the application to Graduate Study. If you do not meet this deadline you may have your secured position offered to an alternate candidate.
- You will be charged a $500 deposit associated with accepting your secured position in the DPT program. If you matriculate to the DPT program you will have this deposit applied to your first semester tuition in the DPT program. If you do not matriculate, you may request return of the deposit. Directions for requesting return of the deposit are available on the Program web site.
Retention and Progression in the DPT program
To remain in good standing in the DPT program you must:
- Maintain a cumulative GPA of 3.0 or better AND an individual course grade of C or better in each DPT required course;
- Complete all required competencies with a ‘pass’ or better in each academic term;
- Positive Readiness for Clinical Practice decision from the DPT program core faculty, and in good standing in the program.
Transfer Students
The DPT program will consider candidates for transfer on a case-by-case basis with Admissions decisions and conditions of transfer made by the Admissions Committee. If you are requesting transfer from another DPT program, you should contact the Program Director and be prepared to provide:
- A cover letter stating reasons for transfer;
- A copy of transcripts from all institutions attended;
- A copy of GRE scores from the most recently taken GRE;
- A copy of TOEFL scores if required.
You may not transfer in more than one year equivalent of course credits (no more than 45 credits). This may require you to repeat coursework that has already been successfully completed on another campus. If you are admitted through transfer, you will develop an individualized plan of study approved by the Admissions Committee prior to matriculating into the DPT program.
Additional notes for graduation and licensure of DPT students
- Processes for licensure are determined state to state, however all states require passing the FSBPT licensing examination to be licensed as a physical therapist. Graduation from the DPT program does not guarantee you will pass the licensing examination.
- If you are requesting eligibility for the FSBPT National Physical Therapy Examination you must be in good standing in the program, must have completed DPT 962: Licensing Examination Prep with a ‘Pass’ grade, and be judged as passing in DPT 991 by the Director of Clinical Education at the time of submission of eligibility paperwork.
Requirements for Graduation
To graduate from the DPT program you must:
- Successfully complete all required coursework and competencies of the DPT program;
- Receive a recommendation for graduation from the core faculty of the DPT program.

Description: The Health and Wellness Coaching Graduate Certificate teaches competencies needed to help clients make and maintain healthy changes. The program is designed with busy professionals in mind. It is comprised of four courses offered 100% online via a mix of scheduled live interactive classes and independent learning modules. The program can be taken for continuing education credits (CECs) and/or for graduate credit. Both options meet the standards of the National Board for Health and Wellness Coaching (NHWC) and prepare you to sit for the NBHWC certification exam. Description: The Health Science Education Graduate Certificate is awarded upon successful completion of a series of courses that center on competence and proficiency in teaching health science courses at the middle or high school level. The certificate demonstrates expertise in health science topics and curricular foundations and prepares individuals to educate middle and high school students in the health science career cluster and related pathways.

The Senior Living and Services Leadership Graduate Certificate is offered through the collaborative online MS in Healthcare Administration (MS-HCA) and is an additional and optional credential for MS-HCA students as well as a freestanding certificate program for graduate students that may or may not elect to pursue the full MS-HCA degree. The certificate program is offered fully online, asynchronous curriculum comprised of two MS-HCA foundational courses and two courses focused on Senior Care Administration. The certificate is offered jointly by the MS-HCA collaborative partner campuses: UW-La Crosse, UW-Parkside, UW-Platteville, UW-Stevens Point, and UW-Stout.
